Active Partnerships specialise in providing evidence-based physical activity services for a range of medical conditions and health risk factors.
Prescribed Exercise Plans
Prescribed exercise plans are recommended for people managing specific health conditions or risk factors who want to maximise their results in a safe and sustainable manner. For instance, we can help you reduce blood pressure or weight, manage and prevent low back pain or get back on your feet after illness or surgery.
How our prescribed exercise plans work
Prior to starting an exercise plan, you will attend an initial assessment so we can determine your specific needs. During this time we discuss your goals, current and past activity level and any conditions you are managing. We will also take some measurements of your health and physical status. Then a safe and effective exercise plan is developed which you are comfortable with. The location of your program is flexible and dependent on your needs and preferences. We provide home, gymnasium and community based exercise programs.
Our approach is that exercise should be prescribed like medication. To achieve optimal health results, exercise needs to be performed at the right dose (type, intensity, volume) and progressed appropriately for each person. For this reason, your exercise plan includes health and activity goals, specific exercises with progressions and a regular review schedule. We then typically see you for 4 review consultations over a 3-6 month period or until you are confident to manage and progress your program independently.

Strength and Balance Program
This program is ideal for older people living at home or in a retirement village who want to maintain independence, mobility and balance in the convenience of their own homes.
Program information
The program is a home-based, individually tailored strength and balance program delivered by university qualified Exercise Physiologists. The program is founded on the evidence-based Otago Exercise Program which has demonstrated a 35% reduction in falls and falls related injuries in older people. Other important benefits include improvements in strength and confidence for daily activities including walking, getting out of chairs and managing stairs.
How our strength and balance program works
Following an individual assessment to understand your needs, we prescribe a strength and balance exercise program which you are comfortable with and that you can safely perform at home. We provide you with home exercise equipment and an exercise diary to record your progress. To ensure you safely progress with your exercises and stay on track, we typically see you for 3 face-to-face review consultations over the initial 9 week period. These visits can also be conducted at your home if you live within the Neutral Bay, Cremorne or Mosman area. Other program arrangements can be made to suit individual needs.

Supervised Exercise Sessions
Our small group supervised exercise sessions are held at our Mosman location and are ideal for those people who would benefit from a higher level of supervision to ensure they are exercising most effectively whilst providing extra support to establish a routine.
Our experience has been that a combination of supervised and home-based exercise offers greater health outcomes and improved confidence over the longer term.
